- Identification of the copyrighted work claimed to be infringed
- Identification of the material claimed to be infringing and its page address
- Your name, mailing address, telephone number and email address
- A statement that you have a good-faith belief the use is not authorized by the owner, its agent or the law
- A statement that the information is accurate and, under penalty of perjury, that you are the owner or authorized to act for the owner
- Your physical or electronic signature

Counter-notices (17 U.S.C. §512(g))
If material you provided was removed, you may send a counter-notice containing: identification of the removed material and where it appeared; a statement under penalty of perjury that you have a good-faith belief it was removed by mistake or misidentification; your name, address and telephone number; consent to the jurisdiction of the federal district court for your address (or, if outside the U.S., any judicial district where the publisher may be found) and to accept service from the complaining party; and your signature.
Misrepresentation
Under 17 U.S.C. §512(f), anyone who knowingly materially misrepresents that material is infringing, or was removed by mistake, may be liable for damages.
Privacy, image, property and factual requests

Requests are reviewed individually and are not automatically granted; content may be corrected, clarified, removed or left unchanged with an explanation.
